Name Change Request
We, the undersigned, hereby request that the name NSABIMANA Janvier, son/daughter of Munyungurangabo Alphonse and Uwera, residing at Munini village, Ngara cell, Bumbogo sector, in the City of Kigali, be changed from his current name NSABIMANA Janvier to be called NSHIMIYIMANA Jackson in the National Population Registry. The reason for this change is that it is the name I used since the beginning of my life.
